PII is information that can be used to identify you or relates to you and is collected as part of the process of applying for or enrolling in coverage through the Health Insurance Marketplace®.

    I may only use your PII to:

• Help you apply for or enroll in a Qualified Health Plan (QHP) or other insurance affordability programs (like Medicaid or CHIP) through the Marketplace, or

• Provide you with information about Marketplace coverage or help with ongoing coverage.

    I may not use or disclose your PII for any other purpose without your consent, except as required or permitted by law.

You have the right to:

• Get a copy of your PII that I collect and maintain,

• Correct any mistakes in your PII, and

• Ask me to limit the use or disclosure of your PII.
